AyB processes the data in this form to meet its legal consumer-protection duties: registering, handling and answering it, and keeping evidence of that work.
Required: name, document type and number, details, and at least one of address or email. For a minor, the representative’s name, address, phone and email are required.
Optional: consumer phone, purchased item or service and amount, classification, requested resolution and preferred response method. The receipt is sent to the adult consumer’s email or, for a minor, to the representative’s email.
The form and response evidence are retained for at least two years and may be provided to Indecopi or another authority when legally required. Read the privacy notice.
